The problem, named plainly

The board is never the problem. The board being out of date is.

The client's real requests, the scope changes and the little yeses arrive as email to whoever was on the thread. A human PM with twelve projects cannot retype all of it, so the board drifts.

Discovery

The kickoff questionnaire nobody fills in, replaced by a conversation
